Prime Health Care is a fully licensed, multidisciplinary rehabilitation clinic in Mississauga. For over 20 years, our coordinated team of physiotherapists, chiropractors, registered massage therapists, psychologists, and medical specialists has helped patients recover from car accidents, workplace injuries, and chronic pain — working from a single shared care plan so nothing falls through the cracks.
We file your OCF forms, your WSIB paperwork, and your extended-health claims. We bill your insurer directly. If you cannot travel to the clinic, our therapists come to you — or we arrange transportation when you are ready.
MVA Recovery
Ontario’s no-fault accident benefits mean you pay nothing. We complete your OCF- 1, OCF-3, and OCF-18 and start treatment within 72 hours where possible.
WSIB Treatment
We are a registered WSIB provider. Bring only your claim number — we handle Form 7, the Functional Abilities Form, and return-to-work coordination.
Direct Billing
We bill Sun Life, Manulife, Canada Life, Green Shield, Desjardins, Equitable Life, Industrial Alliance, and most other extended-health plans directly.

You were in a motor vehicle accident
You do not pay anything out of pocket, and it does not matter whether the accident was your fault. Ontario’s no-fault accident-benefit system entitles you to rehabilitation, regardless of who caused the crash. We bill your auto insurer directly through HCAI, complete your OCF-1, OCF-3 and OCF-18 forms for you, and start treatment within 72 hours where possible. If you cannot drive, we will arrange transportation, or treat you at home. -
You were hurt at work
We are registered with WSIB and bill them directly. You bring nothing to the front desk except your claim number. We coordinate Form 7, Functional Abilities Form (FAF), and work-conditioning, and we communicate with your employer’s return-to-work coordinator with your consent. -
You are a lawyer, paralegal, adjuster or case manager
Our assessment team produces defensible, SABS-grounded reports — Catastrophic Impairment, Neuropsychological, Functional Ability Evaluation, Future Cost of Care, Section 44 IME and more. We work with plaintiff and defence counsel. Reports are court-ready and delivered within published turnaround windows.
