Privacy Policy
Prime Health Care is a Health Information Custodian under Ontario’s Personal Health Information Protection Act (PHIPA, 2004) and complies with the federal Personal Information Protection and Electronic Documents Act (PIPEDA) where applicable.

What information we collect
- Identifying information — name, date of birth, address, phone, email.
- Health information — medical history, current symptoms, diagnoses, assessment findings, treatment notes, imaging reports.
- Insurance information — auto policy number, OCF/HCAI claim details, WSIB claim number, extended-health insurer and policy/certificate number.
- Legal information — when a personal-injury lawyer is involved, their contact details and consent to communicate.
- Technical information — website visits, IP address (see Cookie Notice).
Why we collect it
- To deliver clinical care to you.
- To bill your insurer (auto, WSIB, extended health).
- To produce assessment reports requested by a lawyer, adjuster or case manager.
- To meet our regulatory record-keeping obligations.
- To communicate with your other care providers, with your consent.
Who we share it with
- Insurance companies — to authorise and pay for your care (OCF-18, OCF-21, WSIB invoices, extended-health claims).
- Other healthcare providers involved in your care — with your express or implied consent.
- Legal representatives — only with your written consent.
- Regulatory bodies (College of Physiotherapists, College of Chiropractors, etc.) — when legally required.
- Law enforcement or courts — only when legally compelled.
How we protect it
- Paper records are stored in locked filing in secured areas.
- Electronic records are stored in PHIPA-compliant clinic management software with role-based access.
- Staff sign and renew confidentiality agreements; access is logged.
- Records are retained for the period required by the Regulated Health Professions Act and our regulators (minimum 10 years past last contact or, for minors, 10 years past age of majority).
Your rights under PHIPA
- You may request a copy of your record.
- You may request that we correct an error.
- Withdrawal of consent. You may withdraw consent for specified uses or disclosures (some legal exceptions apply).
- Complaint to the Information and Privacy Commissioner of Ontario. The IPC contact is on their website.
