What we treat
Most of our motor-vehicle-accident patients arrive with more than one type of injury. The most common combinations we see:
- Whiplash and other soft-tissue injuries of the neck, shoulders, mid- and low-back
- Headaches, post-traumatic dizziness, jaw pain (TMJ)
- Concussion and post-concussion symptoms — light sensitivity, cognitive fog, sleep disruption
- Sprains, strains, ligament and tendon tears
- Fractures (after the orthopaedic specialist clears active rehab)
- Nerve impingements, sciatica, radiculopathy
- Anxiety, PTSD, driving avoidance, sleep disturbance
- Catastrophic injuries — traumatic brain injury, spinal cord injury, multiple-trauma
Direct billing through HCAI — we handle the paperwork
Ontario’s accident-benefit system is built around three forms. We complete all three for you and submit them through HCAI (Health Claims for Auto Insurance):
- OCF-1 — Application for Accident Benefits. Filed by the patient with the insurer. We help complete and submit it on your behalf.
- OCF-3 — Disability Certificate. Confirms your impairments. Required before the insurer determines your benefit tier.
- OCF-18 — Treatment and Assessment Plan. The plan that authorises your physiotherapy, chiropractic, massage, psychology and other treatment. We draft it within 24 hours of intake.
You do not need to do any of this yourself. Bring the police report number, your insurer name and policy number, and proof of ID. We do the rest.
The three SABS injury tiers, in plain language
The Statutory Accident Benefits Schedule (SABS) sorts injuries into three benefit categories. Where you land determines how much treatment your insurer will cover. We will tell you honestly which tier we think your injuries fall into and why.
- Minor Injury Guideline (MIG) — up to $3,500. Simple sprains, strains, abrasions and minor lacerations that resolve in 12 weeks. If your injuries are not actually minor, we document this with objective findings and file an OCF-18 outside the MIG.
- Non-MIG (Standard) — up to $65,000 for rehab and attendant care, for up to 5 years. Serious injuries: complete ligament or tendon tears, fractures, surgical injuries, and any injury that does not resolve within MIG timelines.
- Catastrophic — up to $1,000,000 for rehab and attendant care, for life. Paraplegia, blindness, severe traumatic brain injury, or 55% whole-person impairment combined. We do the catastrophic impairment assessment when both sides need a determination.
In-home therapy and transportation
After an accident, getting to a clinic is often the hardest part. We solve this two ways:
- We come to you. Our in-home physiotherapy, occupational therapy and PSW team can start treatment in your home from week one.
- We bring you in. When you are ready, we arrange wheelchair-accessible transportation between home and the clinic.
This is covered under your accident benefits and we handle it for you.
Multidisciplinary care — one team, one chart
Whiplash is rarely just a physiotherapy problem, and a concussion is rarely just a neurology problem. Prime Health Care has everyone in the same building, sharing the same chart:
- Physiotherapy — active rehab and manual therapy
- Chiropractic — spinal adjustment, regulated by CCO
- Registered massage therapy — soft tissue, trigger point
- Acupuncture — needle or laser, for pain and chronic conditions
- Psychology and CBT — for post-accident anxiety, PTSD, driving avoidance, sleep disturbance
- Physiatry — for medical oversight on complex and catastrophic files
- Occupational therapy — return-to-work, return-to-driving, attendant-care assessments
Frequently asked questions
Do I have to pay anything?
In almost every case, no. Your auto insurer pays us directly through HCAI. The only out-of-pocket scenario is if you exceed your benefit limit, and we will warn you long before that happens.
Will my premium go up?
No. Accident benefits are no-fault. Premiums change because of tort liability findings, not because you claimed rehab.
Do I need a lawyer first?
No, you can start treatment without a lawyer. If your insurer denies care, your injuries are more serious than first thought, or you need an income-replacement benefit (IRB), we will tell you and can refer you to qualified Mississauga personal-injury counsel.
What if I have already been seen somewhere else?
You can switch providers. Bring your OCF forms and any imaging — we will transfer the file and continue your treatment plan.
How long until I feel normal?
Soft-tissue whiplash usually settles within 6–12 weeks with active rehab. Concussion symptoms typically clear within 1–3 months. Catastrophic and complex cases take longer and we plan accordingly.
