Hurt at work? Prime Health Care is a registered WSIB healthcare provider. We bill WSIB directly, complete your forms, and build a return-to-work plan with your employer and case manager. You bring nothing but your claim number.

We are a registered WSIB provider

If you were injured on the job in Ontario, the Workplace Safety and Insurance Board (WSIB) covers your physiotherapy, chiropractic, registered massage therapy and assessments under the WSIB fee schedule. As a registered provider, we bill WSIB directly — you pay nothing out of pocket for approved care.

We handle the forms

  • Form 8 / Health Professional’s Report. Filed by the treating clinician to open and support your claim.
  • Functional Abilities Form (FAF). Documents what you can and cannot do at work — the form your employer and case manager need for accommodation.
  • Program of Care (POC) reporting. We deliver and report under the appropriate WSIB Program of Care (e.g. POC for musculoskeletal injuries).

You do not need to manage any of this. Bring your WSIB claim number and a photo ID; we coordinate the rest with your case manager.

We assess your functional capacity, replicate the physical demands of your specific job through work conditioning, and communicate with your employer’s return-to-work coordinator — with your consent — so the plan fits the job you are actually going back to.
