How to get here

  • By car. We are at Tomken Rd. and Matheson Blvd. East, just north of Highway 401. Free on-site parking, wheelchair-accessible entry.
  • By transit. MiWay routes serving Matheson Blvd. and Tomken Rd. stop within a short walk. We can advise on the closest stop when you call.
  • Service area. Mississauga, Brampton, Oakville, Etobicoke and adjacent communities in Peel and Halton. In-home therapy and transportation are available to and from the clinic — ask the front desk.

Book online

Our online booking widget (Jane.app) is embedded on the live site. You can pick a specific clinician, specify your preferred language, and choose from same-day, next-day or future appointments. If you do not see the time you need, call (905) 602-8998 — we keep urgent slots open for MVA and WSIB intakes that we do not show online.

What to bring

  • Photo ID
  • If MVA: the police report number, your auto insurer name and policy number
  • If WSIB: your WSIB claim number
  • If extended-health: your insurer card and policy/certificate number
  • Any imaging reports (X-ray, MRI, CT), surgical reports or prior physiotherapy notes
  • A list of medications

For lawyers, adjusters and case managers

To refer a client for treatment, an assessment, an IME or a CAT panel, call (905) 602-8998 or email front1@primehealthcareinc.com with the referral package. We will confirm receipt the same business day and schedule the first appointment within agreed timelines.
